Care Team: The care teams are created to help members in need. Currently Under Construction
I’m looking for the form to fill out for the care team. It’s been a long time since I last updated it and I want to make sure the care team has my current information. I thought I’d find a blank here but if it is here I haven’t found it. Can someone help?

I am working on a better form. It will have the same information but be easier to fill out. It also will be in PDF format and you should be able to save a copy to your computer.

I’ll look forward to your draft copy although Rod and the Care Team Leaders should have the final say on the form. They may have some useful additions or other changes they’d like to see. I assume,when you say I should be able to save a copy to my computer, that you mean I can type the answers instead of hand-writing them. If so, that should be a big improvement. My hand writing leaves much to be desired. Thanks for your efforts. –Edna
